Select your custom form by its Display Name (from step 6). From the "When posting to this folder, use:" list choose: Forms.Right click on your Calendar folder and choose Properties.Publish Form As dialog (click on image to enlarge). At work, our Calendars are shared by default so that we can always lookup where someone is and to book meetings more conveniently.
